To get your program started, you will need to submit all the forms and papers necessary for program implementation in 8 Easy Steps. All items must be completed in order for you to be eligible to receive the Level 1 incentive. View Instructions for each step here.
Step #1: Submit Template of School Contract Form.
Step #2: Submit Match Letter.
Step #3: Submit Student Enrollment/Release Forms.
Step #4: Submit the Class Roster Form.
Step #5: Submit Female Measurement Form and the Male Measurement Form.
Step #6: Submit Baseline Surveys
Step #7: Submit original Sign-In Sheets and Weekly Attendance Report.
